the short answer
You complete a 2-minute online intake, pick a protocol and plan, and save a card at checkout, then finish a few remaining questions, the consent forms, and a quick identity check. A U.S.-licensed physician reviews it, usually within 24 hours. If approved, your card is charged and a licensed U.S. pharmacy ships within 5 to 7 business days. If not approved, there is no charge.
There is no video call and no appointment to schedule. The whole process is written questions on the site, a physician reading them, and a pharmacy filling the prescription.
The step people most often miss is step 3. Checkout saves your card but does not charge it, and your intake does not go to a physician until the remaining questions, the consent forms, and the identity check are done. If you entered a card and then closed the page, that is where your order is waiting.
Here is each step in order, what you need to have ready, and how long each one usually takes.

Realistically, count on about a week from finishing your intake to the package leaving the pharmacy.
step 1: the online intake
Everything starts with the intake on pepvio.com. It is a set of written medical questions: your health history, the medications you take now and their doses, and what you are hoping to address. A physician reads your answers later, on their own schedule. The questions take about 2 minutes.
If you already take the medication through another provider, there is a place to note exactly what you take and at what dose, so the physician has the full picture before deciding what to prescribe. Labs are not required to start.
The intake also checks eligibility from your current location and shipping address, because the physician who reviews your intake has to be licensed in your state and the pharmacy has to be able to ship there.
If you are not sure which protocol fits, the quiz matches what you are noticing to a starting point, and the protocols page lists each one. Where a protocol comes in more than one form (an injection or a nasal spray, for instance), you pick the form at your online visit. The physician sets the dose and the schedule.
step 2: checkout
After the first set of medical questions you choose a plan and enter a card. Two things to know here.
Your card is saved and authorized, not charged. The authorization is a temporary hold that confirms the card is valid. Nothing is billed until a physician approves a prescription, and if that does not happen, the hold drops off on its own.
The plan sets how much ships and how often. A monthly plan renews each month. A quarterly or longer plan ships the entire supply in one delivery at a lower per-month price, so a quarterly order arrives as three months of medication in a single box. The price covers the medication. The physician's review is free, shipping is free, and there is no membership or platform fee. Every Pepvio prescription is HSA/FSA eligible, so you can pay with an HSA or FSA card at checkout. Pay-over-time options are offered at the payment step alongside regular card payment.
step 3: finishing the intake
This is the step people most often think they have already finished. After checkout there are a few more items: the remaining medical questions, one or more consent forms, and a quick identity check. The identity check uses the last four digits of your Social Security number and clears immediately, so you do not need your driver's license or passport with you; uploading a photo ID is optional. Your order is not submitted to a physician until all three are done, so an intake that stops after the card screen sits in place until you come back to it.
The remaining items take a couple of minutes. If you closed the page after entering your card, sign in at portal.pepvio.com and the intake picks up where you left off. The sign-in page has a "Send magic link" option that emails you a login link, so you do not need a password.
Once the identity check and consent forms are done, the intake goes to a physician and the review clock starts.
step 4: physician review
A U.S.-licensed physician licensed in your state reviews the completed intake, usually within 24 hours. They read your health history, your current medications, and your answers themselves, and decide whether the protocol is right for you and, if it is, what dose to start at. The physicians come from Wasef Health, a physician-owned and physician-led telemedicine group whose clinicians practice telehealth full-time and are licensed across all 50 states.
If they need more information, they send you a message through the portal at portal.pepvio.com. Check the portal and your email during that window so a question from the physician does not sit unanswered.

step 5: approval, or not
If the physician approves, they send the prescription to the pharmacy, and that is when your card is charged for the first month. Your subscription date runs from that approval.
If the physician decides the protocol is not right for you, there is no charge. The hold from checkout drops off on its own, and there is nothing to refund. Pepvio emails you either way.
step 6: pharmacy and shipping
The prescription is filled by a licensed U.S. 503A compounding pharmacy; which one depends on the medication. The pharmacy prepares your order and ships it within 5 to 7 business days of approval. Shipping is free on every order.
You get email and SMS updates along the way, including a tracking email when the package is handed to the carrier, and order status is visible anytime at portal.pepvio.com.
The label carries your name and the pharmacy's name, the same as a prescription you would pick up locally. Shipping to a PO box works as long as you can collect the package promptly, because the medication is temperature-sensitive and needs to go in the refrigerator soon after it arrives.
If tracking says delivered and you do not see the package, check with neighbors and the usual drop spots at your address first. If it has not turned up after 24 hours, email support@pepvio.com with your order number and the team works with the carrier to locate it. A package lost in transit is replaced.
step 7: what arrives
The box holds your medication, everything needed to take it, and storage and use instructions specific to your protocol. Injectable protocols ship as vials with supplies rather than prefilled syringes, and your physician's dosing instructions tell you how much to draw and how to give it.
Compounded peptides are temperature-sensitive. Refrigerate the medication soon after delivery, ideally within a few hours, and keep vials in the refrigerator (not the freezer) between uses. A multi-month order goes in the refrigerator in full.
If anything in the instructions is unclear, message your provider through portal.pepvio.com before your first dose. They answer technique and timing questions directly. If the package arrives damaged or with a broken seal, do not use it: photograph the damage, email the photos to support@pepvio.com, and keep the original package in case the pharmacy needs to inspect it. A damaged or defective shipment is replaced at no cost.
step 8: refills, changes, and cancelling
Your subscription renews monthly from the approval date, and your plan's regular price does not go up at renewal. Each of the changes below is made from your account at portal.pepvio.com, with no phone call.
- Pause. Pausing keeps your account active and your protocol history in place, and your provider can see you have paused. Resume whenever you are ready.
- Skip a shipment. Skip or delay a single shipment without cancelling, useful if you are traveling or have medication left from a previous cycle.
- Change protocol or dose. Message your physician in the portal. They evaluate the change against your history, and it usually takes effect on your next refill.
- Cancel. Cancel anytime. Cancellation takes effect at the end of the current billing period, so there is no further charge.
- Update your card. Account settings in the portal. If a payment fails, it is retried automatically over several days and you get an email with a link to update the card.
Compounded medication cannot be returned once shipped, for safety reasons, so shipped medication is not refunded; that includes a multi-month supply that has already been delivered. Damaged or defective shipments are replaced at no charge.
