The signal

Your body makes less growth hormone as you get older. The pituitary itself still works fine; it just gets fewer instructions to release. By your 40s the nightly pulses are a fraction of what they were at 25, which is part of why deep sleep and recovery decline with age [1].

How it works

Sermorelin binds the same receptor your natural releasing hormone uses, so the pituitary releases its own growth hormone in normal pulses, mostly at night. Your body's feedback system still decides how much comes out, the same way it always has.

What the research shows

Human studies going back to the late 1980s show it raises growth hormone and IGF-1 levels, including in healthy older adults: a 1992 trial in men over 60 restored IGF-1 to young-adult range within two weeks [2], and a 16-week trial in adults around age 70 confirmed the effect held with longer use [3]. Doctors have prescribed it since the 1990s [4]. Most of the studies are older and smaller than what you'd see for a modern drug.

What people get wrong

People mix it up with growth hormone injections. Growth hormone replaces the hormone directly and overrides your feedback loops. Sermorelin makes your own gland do the work, with the normal limits still in place. That difference is why plenty of doctors who won't prescribe growth hormone are comfortable prescribing this.
